Instructions: The Drug Abuse Screening Test was designed to assess whether or not your level of drug use places you at risk for developing substance abuse disorder. It is important to note that the DAST is only a screening test and cannot be used to diagnose a substance abuse disorder. Only your doctor can provide you with a diagnosis. High scores on this screening test indicate the need for further consultation with a trained health professional, such as a doctor, psychologist or addiction counsellor.

1. In the past 12 months, have you used drugs other than those required for medical reasons?

2. Do you abuse more than one drug at a time?
